Not doing #2 is the cause of the issue. Your copy of services.xml is going to be the one that comes with V4 and if you upgraded it would be the (edited or not) one that came with an earlier version.

You have a resolver file dating to V3 and a services.xml from V4 and that combination means you get duplicates unless you remove all the now-superfluous AttributeEncoders or start over.
