FWIW I started on the SP side and my reading of the code for the Handler of type AttributeResolver is that the NameQualifier and SPNameQualifier attributes are always going to be sent (I did not trace through all of the way to the NameID constructor so it is possible some really clever query string parameter might still result in a NameID without those attributes, but I think it us unlikely).

Yes, the IdP is a Shibboleth IdP. I will try to get some logs tomorrow.
