Many things. Fundamentally, it’s either people directly accessing some of the /Authn/XXX servlet URLs that are sitting in the middle of things without the right pre-conditions, hitting the back button, and/or a failure to set and replay the JSESSIONID. Same as with all the flow exceptions failing to recover the state of a request.
