Probably. It doesn't seem likely to be coincidence that you have a client dropping the connection and preventing the server from responding just prior to it.

None I know of. The log is simply indicating that the flow is terminated with an error view (likely the one rendered when the server can't respond) and then client is trying to continue it. I don't know how to reproduce that. Nothing obvious that I tried worked.
