Issue with attribute release consent

I'm having an issue with consent configuration. Nothing consent
related uncommented in idp.properties except for
"idp.consent.compareValues = false" which I believe is the default
anyway.

When I access any SP after not visiting it that day, the Information
Release screen is displayed. If I accept, logout and access that same
SP again I get redirected to login, but the Information Release view
is not displayed. When I come back the next day, I get prompted again.
I had a few people complain to me about this issue, but I never
noticed because I'm focused on the IdP and less on the services which
I interact with sporadically unlike some of our staff. We still are
ramping up on the number of internal services using SSO so it's not
clear to me how long this has been an issue.

We will likely turn off consent for internal apps but will probably
leave it in place for services outside the organization, but it would
be good to understand why this is occurring before we disable it for
internal services.

We are running 4.2.1 tap image but rebuilt with OIDC and Duo plugins installed.

What can I do to debug this issue further?
