I fixed the one missing colon that broke the example in the IPAddress page, and I'll fix the rest if I can today.

As I said, it's really not "typical" to ever need to use those examples. They're there as a backstop in case somebody is doing something really odd, but you're misconstruing how this 4.1 stuff works if you're thinking you need to do something with it. You want to get rid of general-authn.xml, not put new stuff into it.
