It doesn't really matter whether it is or not, the module automatically handles them. The only way you can get a 404 that I know of is to manipulate the system out of believing its handlerURL is a match for the request. It defaults to /Shibboleth.sso so there's essentially no way to get a 404, and I can't explain how you could break it enough to cause that.

Proxying is the general reason, as Peter suggested.

The other cause is setting handlerSSL to true and accessing it over http.
