No, it means there are large-scale changes to it that affect certain customization strategies. There are no circumstances under which copying or modifying that file would have ever been allowed or supported and doing so is a practical guarantee that an upgrade will break.

Nothing does, it was modified and continues to be a system file implementing the password flow.

That would mean the upgrade didn't apply.
