My suggestion is to treat that as "default" and the absence of a signal. It's counterproductive to tag "everything", or anything that is the majority of cases, it's the other stuff you want to tag.

But the answer is that the <ConditionRef> element would just contain "shibboleth.Conditions.TRUE" if you want to have a condition that always applies.
