DUO Multi-factor on IIS8.5

Anybody out there able to successfully implement DUO 2-factor on a Windows Server / IIS running shibboleth ISAPI?

We have 2 issues:

1)      We're having an issue where after 20 minutes or so we get 403 forbidden to previously authenticated users.

2)      Users who previously shibboleth-only authenticated on one application on the same server switches to a 2-factor application don't get the DUO prompt screen.

I'm hoping to see someone's shibboleth2.xml file to compare.
