Office 365 login error

I have installed and configured shibboleth IDP and it is working fine with my
federation SP. I have setup fedarated domain in Office 365 and it is working
buy with problems:

some of my users are trying to login and they get 800478B9 error on
microsoft login page (strange thing is that they can login once or twice and
third time they get the error), others login without any problem.
I am using shibboleth 3.2 IDP and i do not get any errors in log file.

Any help ?
