We're using Apache Tomcat 7.0.50 as our servlet container. It's running behind an Apache web server using a a proxy. I'm not sure where it came from since I didn't install it.
OS: RHEL 6
Java: jdk1.7.0_02

One interesting thing: the first time I tried to replace the logout-flow.xml, I messed something up and got an exception when I went to /idp/profile/Logout. The page that the exception was on was loading the CSS and images correctly. I'm not sure if that helps, but I thought I would mention it anyway.

Yes, that's correct. Then I have no idea offhand what it is.
What is the container and where did it come from?
