IP Based Activation Condition

Wouldn't surprise me if it was the reverse proxy IP you're seeing in the web server - we put our moodle behind the same reverse proxy as our IdP, and that is only picking up internal IP addresses (all external traffic shows as 'from' the internal IP address of the reverse proxy). That's Forefront TMG for the record.
