IdPv3 Issues with non-closed browsers

Just a quick follow up on this: the issue specifically presents when a user with a logged in session has timed out using the same browser session, then goes to re-authenticate. Page will time out with 500error, but refreshing each step (initial request, then it seems to hop to the page that renders the login page, which also requires a refresh). How refreshing fixes anything, I don't know, why the issue even happens, no clue in logs. Any idea as to why an issue would present like this?
