What's the IdP version. If it's old enough, I'd check the IdP log for an 
indication it's sending an essentially empty assertion to the SP. That 
used to happen under certain cookie screwy conditions if the session 
cookie at the IdP disappeared at the right point. Now it returns a SAML 
error to the SP and logs something at the IdP about it.
