They're not, that's just how IIS is. Its GUI will lie to you about the settings that it actually has in place.

That is the issue, the filter half isn't running for the site you're accessing. If installing it at the site level doesn't work, try globally, and vice versa. Until the filter runs, that message will be present any time the extension part is run.
