Thanks again Scott. I did find that I had in fact altered the conditions flow in my original attempt to interrupt a successful login with the security question task. After you all helped me get the intercept going, I had failed to roll back to the original transition to 'DisplayUsernamePasswordPage' in the conditions flow. That was definitely causing the error.
I will certainly take a look at your recommended reading. I am still a little green with SWF, having only one other experience with it, customizing the CAS login flow, way back when, but I'm getting there.

You will want to take a look at the SWF docs where they discuss flow inheritance, as some of that gets a little odd, but that's the key mechanism that lets you "hook" the login flows without modifying them by putting things into the conditions flow.
And I think it's only the Password login flow that actually inherits from that, since the others are somewhat "non-UI" in nature or are running outside SWF (the external ones).
