RequestMap doesn't seem to be triggering Shib processing

That's why I would advise it never be used except on IIS.

I don't know what you mean by startup; there is no logging at startup that
matters, what matters is what it logs for the request.

Either the hostname is wrong, or you have an Apache setting somewhere
that's overriding the setting in the map.
