More hints on MCB token auth?

I'm happily using MCB with Duo, but for documentation & training purposes, I'd like to stand up an instance that demonstrates user/password auth plus, if and only required by assurance level, an OTP check to a dummy RADIUS server. I'm imagining a user experience similar to MCB Duo, but only offering the "Passcode" option. I see the U Toronto mcb-spring.xml fragment, but don't know where TokenLoginSubmodule would be configured. Pointers? Or is Duo the only "stacked" module available?
