Then the error means what it says. Given the subsequent logging, the
browser is dropping/not setting the login context cookie needed to pick
back up with the login process. There's nothing you can do about that from
the IdP side, it's a client issue. Could be the browser, or could be
something in the way the login is being requested.
Is there any chance the login page on the IdP is being framed in content
hosted by the SP or something else?
