Problems with IdP proxy

There are no changes to any URLs.  The proxy module is simply sending the 
request to a different idp host.  Possibly your custom proxy module is 
affecting the URL somehow.  Apache's mod_proxy doesn't.  You might step up 
logging on your IdP and the IdP's complaint about the URL in more detail.
