App pool crash / filter unloaded - ShibSP 2.5.1 on Windows 2008 Server SP2

greetings all,
working on a new install of the SP on the aforementioned platform. after installation and initial configuration, observed that Shib was not protecting the path specified in shib.xml
further investigation revealed that the ISAPI filter was unloading after the crash shown below:

A process serving application pool 'DefaultAppPool' terminated unexpectedly. The process id was '3648'. The process exit code was '0xff'.

I can trigger the crash by recycling the application pool.
After searching and reading, I tried opening up permissions on the c:\opt\shibboleth-sp folder to allow global write-access. Didn't help.
Tried applying the patch that is on the download site alongside 2.5.1 release. Didn't help.
After reviewing all the installation notes for IIS7 and double checking the setup of the filter, handler mappings, etc., it would appear to be an issue in the filter. Checked the project JIRA but all the app pool related issues are closed.
Has anyone gotten the SP 2.5.1 to run in this environment? Any directions I should be looking in?
I am thinking of uninstalling and trying a previous release over the weekend.
