new to shib: incommon affiliate or roll-your-own?

That gives you the understanding to go forward. :-) 

Quick answers to your 'a' question. In general you will want to run an IdP 
on a separate dedicated host. This is again usually a public facing utility. 
The SP usually lives with the web app it is protecting, either Apache
based or IIS based.

Also do check lists and wiki.. lots of history and info there.
