How is endpoint hostname generated?

Hello,
I have an Shibboleth SP installation that generates different endpoint urls
depending on where it is accessed from: one if accessed from internet,
another when accessed from intranet.
In both cases SP is accessed using the same url in the browser address
line. handlerURL is set in relative form (handlerURL="/Shibboleth.sso").
If I set handlerURL  in absolute form (handlerURL="https:/
my.domain.com/Shibboleth.sso") it works in an expected way.

What is the logic behind hostname discovery? Wiki says it is guessed from
the VirtualHost, but I would like to know details, because I only have one
VirtualHost set up in apache and it is obviously same for both intranet and
internet.
