The SP is probably not the right solution in that case. It's targeted at
the web server administrator, not somebody working on top. It isn't trying
to be all things, I focus on non-hosted scenarios (or hosting where you
just have full access).

But it sounds like you would have to use the map, yes, and if it's not
working, then you have a problem with the web server's config. That's why
it's targeted at the person with control over that piece.

Though in fairness, your web server is simply not operating correctly if
it's not reporting the hostname accurately for requests. That's
independent of the SP. There are just a lot of apps that don't care, and a
lot of misconfigured web servers that are left broken. You can't get away
with that here.
