I confirmed everything at https://wiki.shibboleth.net/confluence/display/SHIB2/NativeSPWindowsIIS7Installer (sorry on IIS7 misspoke earlier). The only IIS6 Management Compatibility not installed are WMI and Scripting tools. Would those make a difference?

native.log says:

     isapi_shib_extension: status handler request blocked from invalid address (xxx.xxx.xxx.xxx)
