Account lockout customization

Happened to notice two different class names mentioned in the trace 
(custom.AccountLockout.ServiceLockoutManager vs. 
andovercos.AccountLockout.ServiceLockoutManager), perhaps there is an 
inconsistency somewhere in the config or jar?
