missing attachments in Jira (XSTJ-5)

Thanks Peter, this has been fixed.  As best as I can tell this was the
result of an error in the Jira upgrade process from 4.2.2 to 4.2.4.
For some reason the attachments for 3 issues were lost.  I have
restored them from a backup.
