When using either FilesystemMetadataProvider or FileBackedHTTPMetadataProvider it's possible to (accidentally?) load two different blobs of metadata with the same entityID which is not allowed.

There's then a chance that the "wrong" metadata will be queried during a flow leading to confusing errors and unexpected behaviour.

When such metadata is loaded, any entityIDs should be checked for duplication and a warning/error raised.
