This is for IdP v2. Just making sure you realize that that will be
fully unsupported as of July 2016. For a new project, you probably
want to base on IdP v3, which is completely different architecture.
For an existing IdP, well, you probably want to think about
migrating to v3 really soon...<br>

I'm not sure, I would recommend tracing the request(s) in the
browser with something like Firefox Live Headers to see the exact
sequence. Might give a hint as to what's going on.<br>
