Un-scoped Microsoft IdPs and ePPN

At least for SP4 (and when proxying), IdP 5.2 will include a Scope metadata filter, and I'm making some improvements to other similar filters that solve additional problems with them I've hit over the years.

I will say that we really need people to file issues. My days of being the deployer of last resort who can identify these sorts of improvements is coming to an end, so the community has to be the one picking up the <sic> slack on that.
