I think your threshold for noticing things is probably higher than mine :), I get a daily log report of any anomalous or unexpected output, so if something unusual happens even once I notice. This happened twice in the last couple of weeks, the first one I looked at sideways and let go, the second one I decided to investigate.

Well, it turns out you are right, but not in the way you thought. The node of my test cluster experiencing this error turned out to be still running 4.2.1 8-/. I'm not sure how that happened, they should've all been updated to 4.3.1 back in April.
When I was testing, I thought it was sporadic and that I had successfully processed logins via this node, but it turns out it failed every time.
Bottom line, I screwed up, sorry for wasting your time…
