Additional Entity in IIS

One additional thing I am seeing is with Apache the $_SERVER['Shib-Handler'] in phpinfo.php that resides on the second entity location is the second entity path while in IIS it is pointing to the first entity path.

Thanks for the clarification Scott.
This is very strange, I am using the identical settings but with IIS it does not work.
Maybe it is something with the way I have the RequestMap configured? I spent a few hours trying different settings with no luck.
