IIS Module not setting Cache-Control

Most of the code was copied/ported pretty blindly, and if it's not setting the headers, there may be a reason that won't be that simple to work around. Hopefully it's just an oversight.

That's going to be our interim approach through web.config, but we would obviously rather not keep that in place if possible.  There will be a little collateral caching damage to other resources unless we can figure out the syntax better, but not enough to cause widespread application of the header.
