Duplicated attributes in SAML response

This has also been covered on the list before. IDPv4 comes with an
"attribute registry" (activated by default for new installs, but not
for upgraded systems) that provides standardised "metadata" for
attributes, including AttributeEncoders.

So when adding (or copying over from IDPv3) your own attribute
definitions to your resolver that match the id of attributes covered
by the attribute registry those should not include their own
AttributeEncoder elments.
