Logout notifications always red X (failed)

Hi all,

when performing SLO on Shibboleth 4.1.4, logout propagation completes
sucessfully and user is correctly logged out of all services, however, the
final /idp/profile/Logout webpage always shows red X for all services.
Shibboleth 3 reported correct logout status.

idp-process.log lists Success and no errors:

Any ideas what needs to be done to properly show logout status on the
Logout webpage?
