Does anyone have any suggestions as to how we should investigate this issue and track down the problem?
Look at the ACS being requested in the SAML request (or just use the one from the logs) and compare to the installed sp metadata ACS list.
We see vendors change their host domain and that generally breaks the SSO flow because the new domain isn't on the installed ACS list.
