Thanks, Scott. Yes, I reproduced it exactly as you described. Looks like he either used the back button or has something seriously messed up in his browser configuration.

If you managed to reproduce it without just ending up triggering the exception outright before ever getting the chance to even resubmit the form.
If you want to see the exception, just do a normal failed password and hit Back, and watch the log. You should get that exception, not the NoSuchFlow exception.
How you could manage to get it on an actual submit, I can't say. Possibly browser caching that causes it to redisplay the previous page state and URL without hitting the server so that it ends up causing the exception only on the submit step.
