SP Headers missing attribute

I had the exact same issue, where Shibboleth got me to login to IDP, but on
return to SP in IIS there were no Shibboleth-specific request-headers
present. After a lot of digging I found that the ISAPI-filter was not loaded
for the site. (C:\opt\shibboleth-sp\lib64\shibboleth\isapi_shib.dll)
