That's half, but if shibd is what's actually crashing that's the half that would be in those logs. If Apache is, it's the other half and not those logs.

Very unlikely but if there's a specific account then you should be able to get a trace of messages and have a reproducible bug.

You configure the Linux OS to get them produced when processes crash, I have no particular knowledge of any of that, it's been a long time since I looked into it. I think there's a system variable involved that caps core dump sizes and uses zero to essentially turn them off.
