SAMLtest.id updates

All,

 SAMLtest now allows you to specify an arbitrary NameIDFormat or authnRequestContextClassRef on the IdP test initiation page under "Advanced Features".  I resisted calling the button "vendor emulation mode".
 There was also a brief outage this morning.  Some invalid metadata made it through, but I believe only two testers should have noticed.  I've fixed the metadata in question and added another schema to validate against during upload so that that particular one won't happen again.
