SP behind proxy - ServerName not used

ServerName should be sufficient to address all this, and it always has been as far as I'm aware. If you're involving headers and trying to do something with them, perhaps that's interfering and perhaps overriding things. Otherwise the usual explanation is that it's falling into the wrong vhost definition.

Obviously the best thing, which you probably already did, is turn off the SP and ignore that part until SERVER_NAME and SERVER_PORT are correct for every request.
