Rewriting, maybe a redirect in the app before the SP is invoked, a bug in the code that's actually integrating the SP with the application if it's a case of manually invoking login requests. All of them involve the application. The SP just does what it's told and processes the URLs it sees, so that's what it saw at the point it issued the request, that was the current URL or the target value passed to a SessionInitiator endpoint (/Login).
