Does it log it at filter startup adding a mapping to that path? Should say something about it.

Otherwise if it's not seeing them, something else is stepping in and preventing the filter from ever seeing the requests.

Maybe that's a virtual mount to some weird .NET thing I probably don't know anything about, but probably involves AppPools. My go to explanation for everything I don't get about IIS is that it's an AppPool problem. That's half serious.
