I'm very skeptical, so all I can say is you can file a bug and I'll eventually try and reproduce it and if I can't, I'll invalidate the bug report. My guess is perhaps you aren't using the configuration you think you are, but since virtually nobody turns this off, anything's possible.

If it were broken, it's also not as though there's anything I could do about it soon, so you're probably going to need to look at hardwiring the web server to report a value in a header and then direct it to pull REMOTE_ADDR from there, which should work around the issue.
