Without staring at it endlessly, I would say there's probably a mistake on one side or the other in terms of the URIs involved, and if not, then the only explanation would be that you didn't restart shibd or at least do a full config reload. The attribute mapping file doesn't reload itself by default in at least most recent versions.

You're also running an out of date version, FWIW.
