Change in NameQualifier default handling

I guess this was pointed out by somebody recently asking a question about it but I didn't pick up on it just then. I (finally) got around to converting my NameID handling over from the resolver to the generators and noticed that there was an inadvertent change to the default handling of NameQualifier/SPNameQualifier from not setting them to setting them.

That was accidental, but it's a bit late to do much about it now, and it's not a change to any existing upgraded-from-V2 behavior, just that the new behavior doesn't match the older.

I added omitQualifiers="true" to the default config example and I'll add it to the documentation as well just so that the usual result for a custom format is the same as it was in V2.
