Debugging authn/Duo IdP3.3.0

I am working through implementation of MFA with a Duo transition.  duo.properties are all set.  I am at DEBUG log level on 'idp.loglevel.idp' in the logback.xml.

The Duo factor is failing somehow:
DEBUG [net.shibboleth.idp.authn.impl.TransitionMultiFactorAuthentication:197] - Profile Action TransitionMultiFactorAuthentication: Applying MFA transition rule to exit state 'authn/Duo'
DEBUG [net.shibboleth.idp.authn.impl.TransitionMultiFactorAuthentication:219] - Profile Action TransitionMultiFactorAuthentication: MFA flow completing with event 'AuthenticationException'
WARN [org.opensaml.profile.action.impl.LogEvent:105] - A non-proceed event occurred while processing the request: AuthenticationException

Logs are not showing anything about what the AuthenticationException event is referring to, so I am having trouble figuring out what is wrong with my Duo config.

Any thoughts?
