Based on a quick read of SWF code, what I think triggers it is likely users getting tired of waiting and reloading pages, causing requests for a given conversation to back up waiting on the hung request. Just my guess at this point.

I believe the timeout is probably configurable, but 30 seconds is the default and that's already too long for the IdP of course.
