My file was forked from the repo you linked, which was indeed originally authored by "andy at tagish.com", which they published for free (according to some elusive wikipedia links).
At this stage the code exists win multiple forms in multiple repos and I don't intend to add to that number.
No other modules outside of this .jar are required to get the JAAS DBAuth to work.
